Advance Care Planning. Join us at the Squamish Public Library at 6:30pm to learn more about advance care planning and how to your goals and wishes for your life and health should there be a time when you can't speak for yourself.

Memory Tree Event. Join us for our annual Memory Tree Event at On the Farm Shops. Gather with friends and family and be supported as we remember and honour the memory of our loved ones at the start of the holiday season.

Palliative Care

Through support, Palliative Care helps patients shift their focus; it is designed to ensure patients live the best they can given their diagnosis. Palliative Care can support those with Chronic Illness, Incurable Diseases as well as supporting those facing end of life.

Palliative Care Team

Squamish Hospice Society now has a Palliative Care Team lead by:

Dr. Judith Fothergill - Physician Lead 

Jane Perepalkin RN  - Palliative Care Coordinator

The Palliative Care Team also includes: Home Care Nurses, Community Pharmacist, Occupational Therapist, Home Support Workers and Hospice Volunteers.

Support

Referrals for Hospice support may be made by patients and their families, friends, family physicians, social workers or home nurses. Admission to Hospice will be at the consent of the patient.

In response to requests for service, a coordinator will arrange to visit and begin to coordinate your support programs.
